Passport Services

The City Clerk's Office provides Passport Acceptance services. No appointment necessary. Please click here to view instructions on applying for a US passport. Early arrival is encouraged. To apply for a passport, please follow the instructions below:

 

Services will be provided:
Monday through Thursday - 7:00 AM to 6:00 PM (except holidays)
Closed every Friday, Saturday and Sunday.

Please bring the following information with you:

  • Original certified birth certificate ( All documents entitled  "abstract" are unacceptable) or naturalization papers.
  • One passport photograph (can be taken at the City Clerk's Office).
  • DS-11 Form (available at the City Clerk's Office or via the website link below).
  • Valid driver's license.
  • Payment by check or money order. No credit /debit cards accepted at local passport agencies.

Requirements for Obtaining a Passport

  1. Appearance: Apply in person to complete the application or you may click here to apply online prior to arrival. TYPE OR PRINT CLEARLY USING BLACK ONLY. All Minors Must Appear in Person with both parents when applying for a Passport.

    DO NOT SIGN THE APPLICATION UNTIL ASKED TO DO SO.

  2. Proof of U.S. Citizenship:
    1. Certified copy of Birth Certificate (California short abstract is not acceptable)

    2. Prior Passport
    3. Naturalization Certificate. (Only Certified copies showing an agency seal are acceptable).

    NOT ACCEPTABLE: Hospital Certificates, Baptismal Certificates, Notification of Birth Certificates, or photocopies of Certified Certificates.

    Original Documents proving U.S. citizenship must travel with the Passport Application. Originals Documents will be returned by the U.S. Department of the State after the passport is issued.

  3. One Passport Photo:

    Photo must be 2" x 2" in size, color, with a white or off-white background. Regular candid photos and do-it-yourself booth photos are NOT ACCEPTABLE. A Passport photo may be taken at City Clerk's Office at a cost of $12.00 for one photo.
  4. Picture Identification:
    1. Current valid driver's license.
    2. California Identification Card.
    3. Military ID Card. Minors will use the parent's ID.
  5. Fees

    • Routine Services (your passport in 6 Weeks):
      • Adult Passport - age 16 and over (valid for 10 years from the date of issuance)
        $110.00 payable to US Department of State
        $25.00 payable to the City of Rialto (Processing fee)
      • Child Passport - age 15 and under (valid for 5 years from date of issue)
        $80.00 payable to US Department of State
        $25.00 payable to the City of Rialto (Processing fee)
      • Photograph: $12.00 payable to City of Rialto
    • Expedited Services (your passport in 2 to 3 weeks)
      • Expedite Fee: $60.00 payable to US Department of State 
      • Express Mail Fee: $14.85 payable to US Department of State; includes tracking
  6. Passport Card - The Passport Card is a wallet-size card that can only be used for land and sea travel between the United States and Canada, Mexico, the Caribbean, and Bermuda.

    Fees

    • Routine Services (your passport card in 6 Weeks):
      • Adult Passport Card - age 16 and over (valid for 10 years from the date of issuance)
        $30.00 payable to Passport Services
        $25.00 payable to the City of Rialto (Processing fee)
      • Child Passport Card - age 15 and under (valid for 5 years from date of issue)
        $15.00 payable to Passport Services
        $25.00 payable to the City of Rialto (Processing fee)
      • Photographs: $12.00 payable to City of Rialto
    • Expedited Services (your passport in 2 to 3 weeks)
      • Expedite Fee: $60.00 payable to Passport Services
      • Express Mail Fee: $14.85 payable to Passport Services; includes tracking
